Cancellation and Liability Waiver

Cancellation Policy
Cancellations before May 1, 2015 will receive a refund of the tuition minus an administration fee of $75.00. Cancellations after May 1, 2015 will receive a credit for a future program minus an administration fee of $75.00. Cancellations within 30 days of the start of a scheduled camp are non refundable. All cancellations must be in writing. Refunds will be processed after the completion of the summer season, and will not be issued to no-shows or to players who leave the program by their own admission, by expulsion, or who cancel after the program begins.

Liability Waiver / Consent to Treat
I agree that I shall provide health insurance to cover any personal injury and property damage sustained by the student/s while participating in any PV-Hockey activities or while on the premises of the PV-Hockey camps; by selecting “yes” below, the undersigned assumes all responsibility for any and all risk for damage or injury that may occur to the above named student/s as a participant in any PV-Hockey program, including practices, games, skill sessions, clinics, and other activities related to the program. In consideration of such, the undersigned hereby releases and discharge the program, Paul Vincent, PV-Hockey, it’s operators, employees, agents, supervisors, instructors, and other players from all claims, demands, rights or cause of action present or future, whether known or anticipated and resulting from or arising out of or incident to the undersigned participation with said program.

I give permission to have my child examined and treated by a qualified medical professional if he or she becomes injured in my absence.

Discipline Policy
1. Discipline and guidance shall be consistent and based upon an understanding of the individual needs and development of a child.

2. The goal of discipline is to maximize the growth and development of the children and for protecting the group and individuals within it.

3. Behavior which constitutes discipline:
• Campers whose behavior distracts other participants from learning
• Fighting or abusive language on the ice
• Penalty during scrimmage
• Inappropriate behavior

4. Campers who fall into any of the above categories will be escorted off the ice to the bench, at which time they will be spoken with. A staff member will explain why their actions were inappropriate. After the camper understands why their actions were inappropriate, they may then continue to participate with the scheduled activity.

5. If a camper continues to use behavior that is considered inappropriate, a staff member will speak with the parents about their child’s behavior. The staff member will explain the problem and why we do not condone this behavior.

6. If a camper continues to use behavior that is considered inappropriate, after a staff member has spoken with both the camper and the parent, that camper will then be expelled from the camp. A staff member will call the camper’s parents and have them come and pick up their child.

7. The staff member will fill out an incident report, before speaking with the camper’s parents, detailing the behavior(s) and problem(s). All reports will be kept on file.

8. Reasons for dismissal from camp (No refund will be given if expelled from camp):
• Fighting in the locker room
• Swearing at staff members
• Stealing of money, equipment, etc.
• Continued inappropriate behavior

9. Prohibitions:
• Corporal Punishment, including spanking and touching; touching of participants is prohibited
• No camper shall be subjected to cruel and severe punishment, humiliation, or verbal abuse
• No camper shall be denied food or shelter as a form of punishment
• No child should be punished for soiling, wetting, or not using the toilet

PVPDH strictly abides by the rules and regulations set forth by the State Of Massachusetts. The school is properly licensed and is in compliance with these regulations.
